will discuss the importance of engaging veterans through community-based
services. It will highlight examples of successful partnerships to
implement chronic disease self-management education programs developed by
organizations in Utah and Maryland.

This webinar is part of a NCOA series focused on promoting
healthy aging and access to benefits among U.S. veterans. Older veterans
have substantial disease burden as compared to civilians. About half of
veterans are age 65 and older. However, only one-third of veterans age 65
and older use the Veterans Health Administration for health care, and
many use both VA and community-based health and social services. Older
veterans may not be accessing community-based or VA benefits or services
that may help to promote physical and mental health in late life.
