The Cognition in Primary Care Program offers in-clinic training models to primary care providers to facilitate efficient cognitive evaluations in the primary care setting. Launched in September 2021 with funding from the Centers for Disease Control and Prevention (CDC), the program is based on the Gerontological Society of America’s GSA KAER Toolkit. Visit Cognition-PrimaryCare.org to access practical, training-based tools for primary care and the in-clinic model for identifying cognitive impairment to help guide patients and families during a dementia illness.
