CMS Innovation Center Strengthening Equitable Access to Advanced Primary Care Listening Session

The CMS Innovation Center hosted a listening session on Advanced Primary Care on Tuesday, April 26th with 8 guest panelists, representing patient advocacy groups and provider groups, as well as foundation and research experts. The CMS Innovation Center noted the following themes that it will take into consideration for future planning:

  • CMS may consider further opportunities to support primary care practices, particularly smaller and/or under-resourced, in their transition to value-based care by reducing administrative and technical burden and investing in data infrastructure.
  • CMS may consider providing additional support for integration of team-based care delivery, to reflect the vital role of community health workers, specialists, and other support staff in meeting community medical and social needs.
  • CMS may consider development of more meaningful and accurate ways to measure primary care practice performance and population health, focusing less on individual disease and more on social risk factors and practice norms.
