CMS is pleased
to announce the availability of the final MCBS Winter 2021 COVID-19 community
and facility survey instrument questionnaires at the following location: 2021
Winter Supplemental COVID-19 Questionnaires. These survey
instruments were used to collect information from Medicare Current Beneficiary
Survey (MCBS) respondents regarding the impact of COVID-19 on their health and
well-being, as well as to collect information on COVID-19 vaccine uptake.
This is the 3rd in a series of surveys on COVID-19 collected
within the MCBS.
