Today, the Centers
for Medicare & Medicaid Services (CMS) released a notice of funding
opportunity (NOFO) that is intended to award planning grants to states for
activities related to the development of a State Plan Amendment (SPA) for
children with medically complex conditions in order to plan and develop
this health home model.
CMS expects state
implementation of the new health home state plan option to improve care
coordination for children with medically complex conditions. Section 1945A
of the Social Security Act provides an opportunity for states to cover care
coordination and other services that are expected to support a
family-centered system of care for children with medically complex
conditions and could help to improve health outcomes for these
children. By implementing the health home option, states can cover
coordination of care provided by the
full range of pediatric specialty and subspecialty medical services,
including coordination of care and services from out-of-state providers.
