Anthem Blue Cross of California launched...

...a new Health Homes program for its managed Medicaid members in July. The program will identify people who struggle with multiple chronic health conditions, severe behavioral health needs and homelessness, as well as those who use emergency services frequently. The California insurer has 772,093 Medi-Cal members.
