The Accountable
Health Communities (AHC) Model encourages organizations to align
community partners and health systems to address health related social needs.
CMS recently released a case study, Building
Strong Community Partnerships to Address Social Needs (PDF), that
highlights how one organization, Health Net of West Michigan, leverages its
advisory board and partnerships to achieve this goal. The case study includes
successful strategies for engaging community members, partnering across
sectors, structuring advisory board meetings, and taking evidence-based
