The Many Facets of Advance Care Planning

This webinar will cover advance care planning at Southcentral Foundation (SCF), where providers support patients (who are called “customer-owners” at SCF) in coming up with advance directives and taking steps to see they are carried out. Topics covered will include the creation of advance directives, durable power of attorney for health care, living wills, DNR/DNI orders, patient rights, and more.

Objectives:

  • Identify the critical elements of advance care planning
  • Define ways to navigate potential difficulties when working with patients on advance care planning
  • Analyze the ways in which SCF’s relationship-based approach to care facilitates advance care planning
