Provide Input on D-SNP Provisions of the Bipartisan Budget Act of 2018

Section 50311(b) of the Bipartisan Budget Act of 2018 (Public Law No. 115-123)[1] creates a new Section 1859(f)(8) of the Social Security Act in order to increase integration of Dual Eligible Special Needs Plans (D-SNPs). In particular, the statute directs CMS to: 1) develop unified grievance and appeals processes for D-SNPs; and 2) establish new standards for integration of Medicare and Medicaid benefits for D-SNPs. We are soliciting comments to help inform our next steps related to unified D-SNP grievance and appeals processes and new integration standards.
Through April 12, 2018, CMS is inviting written stakeholder input on these provisions. New Report on the Initiative to Reduce Avoidable Hospitalizations among Nursing Facility Residents
The first annual evaluation report of the Initiative to Reduce Avoidable Hospitalizations among Nursing Facilities (The Initiative), Phase Two, was released last month.  You can find the report here:https://www.cms.gov/Medicare-Medicaid-Coordination/Medicare-and-Medicaid-Coordination/Medicare-Medicaid-Coordination-Office/InitiativetoReduceAvoidableHospitalizations/Evaluation-.html
This report explains how the second phase of the Initiative, which tests whether a new payment model for nursing facilities and practitioners improve quality of care by reducing avoidable hospitalizations, will be evaluated and presents some early qualitative findings from the first year.  Quantitative results for the first year will be available in next year’s report.

Resources for Integrated Care is excited to announce the 2018 Disability-Competent Care Webinar Series. This series is intended to aid providers and health care professionals in improving the delivery of care to persons with disabilities. Individuals with disabilities enrolled in both Medicare and Medicaid are at higher risk for poor health outcomes when compared to individuals without disabilities.1 This seven-part series explores key concepts of the Disability-Competent Care (DCC) model and how organizations can best implement these concepts to empower providers and participants with disabilities to best meet their care needs.  The final four webinars in the series will take place Wednesday, March 14, 2018 through April 4, 2018. Continuing Education (CE) credit from CMS’ Learning Management System will be available at no cost to webinar attendees.
Supporting Participants with Complex Behavioral Health Needs, March 14, 2018
This webinar will provide background on the needs of adults with disabilities with complex or co-occurring behavioral health needs. These challenges range from highly variable to severe mental health issues, commonly co-occurring with substance use disorders or other functional limitations. This webinar will examine the innovative approach the Commonwealth Care Alliance (CCA) health plan developed to address the needs of its population. CCA partnered with a local community provider to create a structured living setting as an alternative to institutionalization. This is just one example of a flexible approach to care aimed at improving the accessibility, quality, and care experience for participants and providers.
Interdisciplinary Team Building, Management, and Communication, March 21, 2018
This webinar will explore the value of the interdisciplinary team (IDT) in supporting and empowering participants with disabilities. A strong IDT is necessary for providing care coordination that keeps the participant’s experience and concerns at the forefront of care delivery. The webinar will examine a highly integrated provider and plan IDT, while also exploring strategies to build and support partnerships with local community-based practices. Presenters will provide concrete and innovative strategies to improve the management and communication within the IDT, including meeting execution, agenda setting, and facilitating communication when in-person meetings are not practical. Refining or designing new IDT processes/procedures is a crucial step to improving the participant experience and promoting participant-centered care.
Managing Transitions with Adults with Disabilities, March 28, 2018
This webinar will describe the challenges participants with disabilities experience as they face care settings transitions and steps that providers can take to help empower participants to make informed decisions and improve care outcomes during these transitions. The presentation will help health plans and health systems prepare for successful transitions using innovative approaches to improve the participant experience, increase the quality of care, and build relationships with local and community-based care settings. Presenters will review strategies and resources to facilitate the development of a comprehensive transition plan and steps to ensure that the participant’s choices are addressed during implementation.
Building Partnerships: Health Plans and Community-based Organizations, April 4, 2018
This webinar will explore the benefits of partnerships between health plans and community-based organizations to establish a network with local leadership and support community integration for persons with disabilities. Social determinants of health – such as affordable and accessible housing, transportation, nutritional adequacy, and social and familial supports – may impact participant health outcomes and create challenges for providers and health plans. Partnerships built on bringing together the competencies and knowledge of medical providers and community-based organizations assist with both the transitional needs of participants as well as longer-term support for success in the community. This webinar will highlight how one Center for Independent Living has created such a partnership program in Southern Colorado, with services to engage participants and improve their integration in the local community.
Intended Audience:
This series is intended for providers and health care professionals, front line staff with health plans and provider practices, and other stakeholders interested in improving their ability to meet the needs of adults with functional limitations.
CE Credit Information:
Continuing Education (CE) credit from CMS’ Learning Management System will be available at no cost to webinar attendees. The Centers for Medicare & Medicaid Services (CMS) – CMS is accredited by the International Association for Continuing Education and Training (IACET) and is authorized by IACET to offer CEUs. CEUs will be awarded to participants who meet all criteria for successful completion of this educational activity.
