PHILADELPHIA--(BUSINESS WIRE)--Oak Street Health, a network of health centers providing
primary care for Medicare patients, and leading health and well-being
company Humana Inc. (NYSE: HUM) have entered into a
value-based care network agreement in the Philadelphia area designed to offer a
coordinated, patient-centered experience and help Humana Medicare Advantage
members in Pennsylvania achieve better health.
“As
part of this agreement, Oak Street Health and Humana will continue our
important work together, focusing on strategies to improve the health and
well-being of our Medicare Advantage members”
The
agreement aims to offer quality, cost-effective care to enhance both the
patient experience and health outcomes by giving Humana Health Maintenance
Organization (HMO) and Preferred Provider Organization (PPO) Medicare Advantage
health plan members in Pennsylvania access to Oak Street Health’s first patient
care centers on the East Coast, all located in Greater Philadelphia.
Oak
Street Health currently serves patients at four Philadelphia clinics recently acquired from CityLife Neighborhood Clinics - at
1200 W. Godfrey Ave., 1010 W. Lehigh Ave., 37 S. 40th St., and
6232 Market St. - serving the Fern Rock, Lehigh Avenue, University City and
Cobbs Creek neighborhoods.
In
addition, Oak Street Health will have a total of three other locations in the
city. Two are scheduled to open later this year in Philadelphia – at 5050
Parkside Ave. and 3621 Aramingo Ave. – serving residents of West
Philadelphia, Aramingo and Port Richmond neighborhoods. The third, at 2401 W.
Cheltenham Ave. in Wyncote, is now open to patients and serves residents in
lower Montgomery County, as well as in the Mount Airy and Oak Lane
neighborhoods.
“We’re
proud to broaden our reach nationally and bring our proven model of
personalized primary care to Humana members in Pennsylvania,” said Amyn
Andharia, Regional Vice President of Oak Street Health. “As Oak Street Health
expands outside of the Midwest, we’re pleased to deepen our longstanding
relationship with Humana in ensuring that people with Medicare have additional
access to quality care and receive a great patient experience.”
Oak
Street Health contracts with Medicare Advantage and Medicare Part D Health
Plans, including numerous Humana Medicare Advantage plans. In addition to the
Pennsylvania locations, Humana Medicare Advantage members have access to 40
other Oak Street Health clinics located in Illinois, Indiana, Michigan, and
Ohio.
Oak
Street Health treats people with Medicare and has shown significantly improved
outcomes, providing quality care and reducing costly hospitalizations,
according to a recent case study1. The company provides personalized
experiences and a variety of individual resources for its patients, including a
dedicated care team with a doctor or nurse practitioner, and community rooms at
each center.
“As part
of this agreement, Oak Street Health and Humana will continue our important
work together, focusing on strategies to improve the health and well-being of
our Medicare Advantage members,” said Eric Bohannon, Humana Regional President
of Medicare in Pennsylvania. “By adding these Oak Street Health primary care
practice locations to our Medicare Advantage network in the Philadelphia area,
we’re offering patient-centered care close to home, in the communities where
our members live.”
Oak
Street Health and Humana are proud to continue to team up to deliver extensive
value-based care services, with an emphasis to be placed on:
·
More personal time with health professionals and personalized
care that is tailored to each person’s unique health situation;
·
Access to proactive health screenings and programs that are
focused on preventing illness;
·
Improved care for people living with chronic conditions with a
focus on avoiding health complications;
·
Leveraging technologies, such as data analytics, that connect physicians
and help them work as a team to coordinate care around the patient; and
·
Reimbursement to physicians linked to the health outcomes of
their patients rather than based solely on the quantity of services they
provide (fee-for-service).
Humana
has an extensive and growing value-based care presence. As of June 30, 2018,
Humana has approximately two million individual Medicare Advantage members and
approximately 130,000 Group members who are cared for by more than 52,000
primary care physicians in more than 1,000 value-based relationships across 43
states and Puerto Rico. Humana’s total Medicare Advantage membership is more
than 3.5 million members, which includes members affiliated with providers in
value-based and standard Medicare Advantage settings. For more information,

About Oak Street Health

on Medicare in medically underserved communities where there is little to no
quality healthcare. Oak Street’s care is based on an entirely new model that is
based on value for its patients, not on volume of services. The company is

patients and taking on the risks and costs of their care. Oak Street Health has
been investing in communities since 2013 – providing much-needed primary care
