Collective Medical, a real-time care collaboration network that aims to improve
patient outcomes and reduce unnecessary inpatient care. Fallon says it will
leverage this technology to identify and better support its high-risk members,
ease care transitions and address social determinants of health. Fallon Health
currently serves 252,265 members, with 49% enrolled in a public-sector plan,
including 8,771 Medicare-Medicaid dual eligibles.
