Date/Time: Thursday, September 6, 2018 from 2:00 – 3:30pm (EDT) This webinar, in collaboration with Administration for Community Living, will describe practical person-centered tools and approaches organizations and professionals can adapt to better support aging in place. Person-centered care is consistent with the values, needs, and desires of individuals and is achieved when providers and health plans involve individuals in healthcare discussions and decisions. Speakers will share strategies for integrating person-centered approaches into clinical and community-based settings, and will discuss how approaches differ based on the individual goals of the client. Meeting the Language Preferences of Health Plan Members

This webinar will provide an overview of
strategies health plans can use to assess and meet diverse language
preferences. Health plans serving dually eligible beneficiaries can
play a key role in identifying and communicating language preferences to
front-line staff and providers in their network to help remove language
barriers to care. Speakers will share strategies for collecting language
preference data and examples of how plans use this data to inform health plan
operations, such as providing translation and interpretation services.
Speakers will also share strategies for hiring and training health plan staff

Peer supports refer to mutual
support and encouragement from an individual who has lived experience with a
behavioral health condition and can use that lived experience to help others.
The peer workforce supports client self-management to promote recovery and
resiliency. This new podcast explores how The Council of Southeast Pennsylvania, Inc. (The Council) restructured their peer supervision program by centering their supervision model on strength-based principles to better address the needs of their peer staff and supervisors. The Council - a prevention, education, intervention, and recovery support organization - provides services to help reduce the impact of addiction and support individuals with mental health conditions and other related issues through their wellness journey. The Council’s Executive Director and two recovery support specialists and supervisors share practical tips to make peer supervision impactful for your staff, and to build a supportive and strength-based culture.
