INDIANAPOLIS--(BUSINESS WIRE)--Oak Street Health, a network of health centers providing
primary care for Medicare patients, and leading health and well-being
company Humana Inc. (NYSE: HUM) are expanding the availability of
coordinated, patient-centered care in Indiana. Through a value-based care
network agreement, Oak Street Health’s newest Indianapolis-area location serves
Humana Medicare Advantage members in an effort to help them achieve better
health.
“By
pairing Humana’s extensive Medicare experience and Oak Street Health’s passion
for patient care, we’re aligned in our dedication to serve the community with
the opening of this newest Indiana location.”
The
agreement aims to offer quality, cost-effective care and enhance both the
patient experience and health outcomes in the city. The new Indianapolis
location – at 2240 East 53rd Street (Suite B-1), in the
Glendale neighborhood on the city’s north side – is in-network for Humana
Medicare Advantage Health Maintenance Organization (HMO) and Preferred Provider
Organization (PPO) health plan members in Indiana. Humana’s Medicare Advantage
plans are the only Medicare Advantage plans accepted at the center, which also
accepts original Medicare.
Opened
this month, the Glendale center is Oak Street Health’s fourth in metro
Indianapolis, with locations on the south side of the city (in University
Heights at 4200 S. East Street); on the east side (in Irvington at Irvington
Plaza, 6401 E. Washington Street); and west of the city in Speedway, Indiana
(5926 Crawfordsville Road, Unit B). With the addition of the Glendale location,
Oak Street Health has seven Indiana centers, including locations in Fort Wayne,
Gary, and Hammond.
“We’re
pleased to work with Humana in expanding patient-centered care in Indiana,”
said Matthew McLaughlin, Regional Vice President of Oak Street Health. “It’s
exciting to serve patients where they live on the north side of Indianapolis as
we deepen our relationship with Humana. We share a longtime commitment to
bettering the patient experience and improving health outcomes.”
In
addition to the new Indianapolis location, Oak Street Health has 40 other
clinics located in Illinois, Indiana, Michigan, Ohio and Pennsylvania.
Oak
Street Health delivers care for people with Medicare and has shown
significantly improved outcomes, providing quality care and reducing costly
hospitalizations1. The company provides personalized experiences and
a variety of individual resources for its patients, including a dedicated care
team with a doctor or nurse practitioner, and community rooms at each center.
“We’re
delighted to broaden our network in Indianapolis and provide Hoosiers greater
access to patient-centered care,” said Larry Costello, Humana regional
president of Medicare in Indiana. “By pairing Humana’s extensive Medicare
experience and Oak Street Health’s passion for patient care, we’re aligned in
our dedication to serve the community with the opening of this newest Indiana
location.”
Oak
Street Health and Humana are proud to continue to team up to deliver extensive
value-based care services, with an emphasis placed on:
·
More personal time with health professionals and personalized
care that is tailored to each person’s unique health situation;
·
Access to proactive health screenings and programs that are
focused on preventing illness;
·
Improved care for people living with chronic conditions with a
focus on avoiding health complications;
·
Leveraging technologies, such as data analytics, that connect
physicians and help them work as a team to coordinate care around the patient;
and
·
Reimbursement to physicians linked to the health outcomes of
their patients rather than based solely on the quantity of services they
provide (fee-for-service).
Humana
has an extensive and growing value-based care presence. As of June 30, 2018,
Humana has approximately two million individual Medicare Advantage members and
approximately 130,000 Group members who are cared for by more than 52,000
primary care physicians in more than 1,000 value-based relationships across 43
states and Puerto Rico. Humana’s total Medicare Advantage membership is
more than 3.5 million members, which includes members affiliated with providers
in value-based and standard Medicare Advantage settings. For more information,

patients and taking on the risks and costs of their care. Oak Street Health has
been investing in communities since 2013 – providing much-needed primary care
